14-20-22. (312) Forms for acknowledgment and denial of paternity 1. To facilitate compliance with sections 14-20-11 through 14-20-24, the department of health and human services shall prescribe forms for the acknowledgment of paternity and the denial of paternity. 2. A valid acknowledgment of paternity or denial of paternity is not affected by a later modification of the prescribed form.
