Section 13. Request For Hearing

A. Written and signed: the request for hearing shall be made in writing and shall be signed by the person or an authorized representative of the person against whom the action of the authority is taken. B. Delivery: the request for hearing shall be addressed to the director of the division of health improvement or to any other authority employee indicated in the authority's notice, and it shall be hand delivered or mailed, return receipt requested, to such person.
