Section 14. Inspection Of Records

The following records are not subject to public inspection, and shall be maintained in a confidential manner: A. Health information protected by state and federal laws. B. Materials associated with reviews conducted by the certification review committee, including but not limited to criminal history information. C. Complaints or allegations regarding a CCHW that are not substantiated following investigation. D. Personal identifiers in applications, such as date of birth, social security numbers, home address and phone numbers.
