EXECUTION PETITION UNDER ORDER XXI RULES 10, 30, AND 41 READ WITH SECTION 151 OF CPC

2.             That I got married to Sh. XXXXXXX on 29.10.XXXX at Delhi.
   

3.             After marriage my name is XXXXXXXX and all the records have the same name.
   

4.             That my AADHAR number XXXXXXXXXXX and PAN number is XXXXXXXXXX which are same before and after marriage.
   

5.             I have not obtained the marriage certificate.
   

6.             I state that XXXXXXXXX and the XXXXXXXX is the name of one and the same person and that is myself.
   

7.             I am executing this declaration to be submitted to the concerned authorities for the change of my name in their database / record.
   

8.             I hereby state that whatever is stated herein above are true to the best of my knowledge.
