# INSURANCE DEPARTMENT BULLETIN ## BULLETIN NO. 2009-08 Bulletin of October 1, 2009 TO: ALL COMPANIES SUBJECT: Audit and examination fees for: -- annual financial statement -- application for original certificate of authority -- foreign examination reports -- Approved reinsurers, Managing General Agents and Risk Retention Company filings. OCTOBER 1, 2009 Effective December 31, 2009, the audit and examination fees charged by this Department relative to the filing of the following documents will be as shown: | *Annual Financial Statements: | | | --- | --- | | Mutual Aid Associations and Fraternal Benefits Societies | $225.00 | | Health Maintenance Organizations | $300.00 | | All other Insurers, including Health Care Service Plans | $850.00 | | Examination-Application for Original Certificate of Authority | $1,500.00 | | --- | --- | | Foreign Examination Reports | $ 200.00 | | Application for accepted reinsurer status | $ 250.00 | | Managing General Agent filings | $ 100.00 | | Foreign Risk Retention Group filings | $ 100.00 | THERE WILL BE NO FEES FOR QUARTERLY FINANCIAL STATEMENTS. *These audit and examination fees are in addition to the fees set forth in Section 27-4-2, Code of Alabama 1975. Please make checks payable to "Commissioner of Insurance, State of Alabama" and deliver to Alabama Department of Insurance, c/o Compass Bank, P. O. Box 830707, Birmingham, AL 35283.
