23 CAR § 83-115. Level term coverage

(a) All applications and, where no application is used, all policies or certificates of insurance, written on the level term coverage shall contain a separate statement acknowledging the fact that the coverage is on the level term coverage. (b) The statement referred to in subsection (a) of this section shall provide for a separate signature by the insured other than the signature required in 23 CAR § 83-103(h).
