27 CAR § 30-176. Form of court clerk certificate

When a court reports nonpayment of a judgment to the Office of Driver Services, the judgment certification shall contain, but shall not be limited to, the following information: (1) Date and location of accident; (2) Date of judgment; (3) Amount of judgment; (4) Name of plaintiff and defendant; (5) Amount of damage to personal property of plaintiff; (6) Monetary amount of damage resulting from personal injury or death of plaintiff; (7) Monetary amount for care and loss of services of plaintiff; and (8) Statement that judgment has remained unsatisfied more than thirty (30) days.
