9 CAR § 30-305. Children’s records

(a) The agency shall keep a confidential case record for each child that includes the following:
(1) Demographic information;
(2) Plan of safe care, if applicable;
(3) A complete intake;
(4) Consents, including consent for medical care, and authority to place the child;
(5) Interstate Compact on the Placement of Children, Arkansas Code § 9-29-201 et seq., information, if applicable;
(6) Case plans and case plan reviews;
(7) Copies of legal documents, for example, birth certificate, Social Security card, or court orders, or documentation of its attempts to obtain the documents;
(8) Physical exams and immunization records, or documentation of its attempts to obtain the documents;
(9) Psychological reports, if applicable;
(10) Educational reports, if applicable;
(11) Disciplinary and incident reports, if applicable;
(12) Records of visitation and family contacts, if applicable;
(13) Documentation of casework services and child contact; and
(14) Discharge statement.
(b) Records for each child shall be kept for five (5) years from the date of discharge unless otherwise specified by Arkansas law.
