(a) Location. Forms referred to in these rules are located on the self-service page of the Arizona Judicial Branch website, www.azcourts.gov. These forms meet the requirements of these rules. (b) Modification of Forms by the Superior Court or Parties. A party may not modify Forms 5 through 10 without court approval, but a superior court judicial officer may authorize a party to modify those forms as provided in Rule 45(a). A party may adapt other forms by deleting content that does not apply to a particular proceeding or by adding other relevant content, provided the adapted form includes all the information that applies to that proceeding. Deleting information contained in a form, or failing to complete a portion of the form, constitutes the party's representation that any omitted or unanswered questions or items are not applicable to the proceeding. <Text of subrule (c) effective until emergency amendment by Supreme Court Order No. R-25-0047, contingent upon the adoption of a conforming amendment to AZ ST CJA § 3-302, but no earlier than Jan. 1, 2026.> (c) Supreme Court. The Supreme Court may adopt, approve, or modify forms as provided by Arizona Code of Judicial Administration § 3-302. <Text of subrule (c), effective after emergency amendment by Supreme Court Order No. R-25-0047, contingent upon the adoption of a conforming amendment to AZ ST CJA § 3-302, but no earlier than Jan. 1, 2026.> (c) Amendments. The Supreme Court by administrative order may substantively change, add, or delete a form. The Administrative Director may approve a technical change to a form that increases the form's usability and understandability, corrects an error, or addresses other technical issues.
