R3-11-1010. Change in Operator

Within 30 days after a change in operator, the Responsible Owner shall provide a written notice to the Board that includes: 1. Name of the licensed animal crematory; 2. Animal crematory license number; 3. Name of the former operator; 4. Name of the new operator; 5. Date on which the new operator assumed responsibility for the animal crematory; and 6. An affirmation, signed by the Responsible Owner, that the new operator received training in the safe and proper operation of the cremation chamber and the written procedures required under R3-11-1007.
