R9-10-602. Supplemental Application Requirements

In addition to the license application requirements in A.R.S. § 36- 422 and R9-10-105, an applicant for a license as a hospice service agency or hospice inpatient facility shall include on the application: 1. For an application as a hospice service agency: a. The hours of operation for the hospice’s administrative office, and b. The geographic region to be served by the hospice service agency; and 2. For an application as a hospice inpatient facility, the requested licensed capacity.
