R9-20-205. Notification of Change

A. A provider shall notify the Department in writing at least 30 days before the effective date of: 1. A termination of treatment provision; or 2. A change in the: a. Name under which the provider does business, b. Address or telephone number of a facility where treatment is provided, or c. Administrator. B. The Department shall update the provider’s approval to reflect the changes in subsection (A), but retain the existing expiration date of the application approval.
