R9-5-707. Submission of Licensure Fees

A licensee shall submit the following to the Department, on an annual basis, no more than 60 calendar days before the anniversary date of the facility’s license: 1. An application, in a Department-provided format that contains: a. The licensee’s name, b. The facility’s name and license number, and c. Whether the licensee intends to submit the applicable fee: i. With the form, or ii. According to the payment plan in subsection (2)(b), and 2. Either: a. The applicable fee, as specified in R9-5-206, or b. One-half of the applicable fee in R9-5-206 with the form and the remainder of the applicable fee due no later than 120 calendar days after the anniversary date of the facility’s license.
