A. An applicant for an HME shall successfully pass a Security Threat Assessment every five years. B. An applicant subject to any of the following actions, as defined in A.R.S. § 28-3001, shall obtain a new Security Threat Assessment and HME: 1. Cancellation, 2. Suspension for a period of one year or more, 3. Expiration for a period of one year or more, and 4. Revocation for a period of one year or more.
