(a) A business required to submit a followup emergency notice pursuant to Section 11004(c) of Title 42 of the United States Code shall submit the notice on a form approved by the Office of Emergency Services. (b) The Office of Emergency Services may adopt guidelines for the use of the forms required by subdivision (a).
