Conn. Gen. Stat. section 38a-477, as amended by P.A. 03-57, An Act Concerning Deficiencies in Insurance Claim Information defines what information is necessary for filing a health claim, and renders that bulletin moot. Third-party payers and health care providers are hereby directed to refer to section 38a-477 regarding the information necessary for filing a health claim.
