The application for a permit by a registered well driller shall include an appropriate
map or plot plan, showing the location of the proposed well and the premises on which
the well is located, in relation to roads, intersections, and other permanent land
features. All permit applications shall be signed by a master driller, as representative
of the registered well-drilling contractor.
