Enterprise Risk Report

The ultimate controlling person of an insurer required to file an enterprise risk
report pursuant to section 38a-135(f) of the Connecticut General Statutes shall furnish
the required information on Form F (Appendix F of Sections 38a-138-1 to 38a-138-16,
inclusive, of the Regulations of Connecticut State Agencies).
