As used in this regulation: (a) "Commissioner" means the Insurance Commissioner of this state. (b) "Form" means a life insurance or annuity policy or contract, or application, certificate, rider or endorsement used in connection therewith. (c) "Insurer" means an insurance company licensed by the Commissioner to write life insurance or annuities.
