R.C.S.A. § 38a-640-1
Definitions

As used in this regulation:
(a) "Commissioner" means the Insurance Commissioner of this state.
(b) "Form" means a certificate or other evidence of a contract of accident insurance or
health insurance or of a total and permanent disability contract, or application,
rider or endorsement used in connection therewith.
(c) "Society" means a fraternal benefit society as defined in Section 38a-595 of the General
Statutes.
