The record in a contested case shall include: (1) All motions, applications, petitions, complaints, responsive pleadings, bills of particulars, notices of hearing, and intermediate rulings; (2) The evidence received and considered by the presiding officer; (3) Questions and offers of proof, objections, and the presiding officer's rulings thereon during the hearing; (4) The decision, opinion or report by the presiding officer or the Commissioner.
