(a) Each person who applies for an identification card that the commissioner is authorized to issue in accordance with section 1-1h of the Connecticut General Statutes shall certify to the commissioner that he or she does not currently hold a valid license issued by any jurisdiction or foreign nation. The commissioner shall not issue an identification card to any person who holds a valid license. (b) If the commissioner issues an identification card the department shall maintain a record for the holder which shall include: (1) full legal name and name history; (2) birth date; (3) gender; (4) address; (5) the date of card issuance and expiration; (6) card number; and (7) the date of cancellation, if any.
