Replacement of damaged operator's license, commercial driver's license or identity card

card The commissioner shall issue a duplicate license, commercial driver's license or identity card, to an applicant whose current operator's license, commercial driver's license or identity card, has been accidentally damaged, mutilated or otherwise rendered illegible. Such applicant shall be required to submit an application, surrender the current operator's license, commercial driver's license or identity card, and comply with the requirements of section 14-137-71 of the Regulations of Connecticut State Agencies.
