Application for certificate to operate intrastate motor bus service under chapter
286 of the general statutes on form provided by commission. Special components required
to be submitted as part of application:
(a) Statement of proposed fares and schedules of service.
(b) Map showing proposed route.
(c) Letter of approval from the traffic authority of each town and city where the applicant
will provide service over such proposed routes on local streets.
