Application for certification to operate taxicab service under chapter 287 of the
general statutes on form provided by commission. Special components required to be
submitted as part of application:
(a) Application fee of twenty-five ($25) dollars in cash, check or post office money order
payable to the treasurer of the state of Connecticut.
(b) Statement of proposed taxicab rates and charges and rules governing operation.
