Application for permit to operate livery service under chapter 288 of the general
statutes on form provided by commission. Special components required to be submitted
as part of application:
(a) Application fee of ten ($10) dollars in cash, check or post office money order payable
to the treasurer of the state of Connecticut.
(b) Statement of proposed livery rates and charges and rules governing operation.
