1328. THERAPEUTIC INTERCHANGE

1328 THERAPEUTIC INTERCHANGE 1328.1 This section shall not apply to generic drug substitutions. For generic drug substitutions, see the requirements of § 1326 of this chapter. As used in this section, “therapeutic interchange” means the dispensing of chemically different drugs that are considered to be therapeutically equivalent. 1328.3 A therapeutic interchange shall not be made without the prior approval of the prescribing practitioner. 1328.4 The approval required pursuant to § 1328.3 may be in the form of a readily retrievable, written, documented policy maintained by the pharmacy which clearly indicates that the provider has intended to approve the therapeutic interchange. 1328.5 The patient shall be notified of the therapeutic interchange prior to, or upon delivery, of the dispensed prescription to the patient. The notification shall include: A description of the change; The reason for the change; and Contact information indicating who the patient may contact with questions concerning the change.
