1007 SPORTS WAGERING ENDORSEMENT APPLICATION 1007.1 An applicant for a sports wagering endorsement shall apply on a form provided by ABRA. The form shall require, at a minimum, the following: The name of the licensee; The establishment’s address; The requested number of sports wagering mechanical or electronic devices, along with where the devices will be placed on the licensed premises; and A detailed diagram of the licensed premises.
