4007. VOUCHER APPLICATION

4007 VOUCHER APPLICATION 4007.1 An Applicant shall submit a security camera system voucher application on a form prescribed by the Office. 4007.2 A completed voucher application shall include: The completed application form prescribed by the Office; Documentation from the Department of Human Services verifying that the applicant has been receiving assistance under a program listed in § 4003.1 for at least six (6) months; and If the Applicant is the tenant of a building, permission in writing from the owner of the building, in a form prescribed by the Office, authorizing: The Applicant to install a security camera on the exterior of the building; and Office staff to verify the location and installation and operation of the security camera system from the owner’s property. 4007.3 The voucher application prescribed by the Office shall include a provision by which the Applicant agrees that they will not use the security camera system for any unlawful or harassing purposes. 4007.4 Upon the determination that an Applicant has submitted a complete application, has complied with all the requirements of this chapter, and is eligible for a voucher, and subject to the availability of funds, the Office shall issue a voucher for the benefit of the Applicant. The Office shall provide the voucher, on behalf of the Applicant, to a security camera system installer, who shall be responsible for installing the security camera system at the Applicant’s building.
