4308 ISSUANCE ─ DENIAL OF APPLICATION FOR LICENSE, NOTICE 4308.1 If an application for a license to operate is denied, the Department shall provide the applicant with written notice that includes: (a) The specific reasons and legal authority for the license denial; (b) The actions, if any, that the applicant must take to qualify for a license; and (c) Notice of the applicant’s right to appeal and the process and time frames for appeal as prescribed in Chapter 48.
