2555. ADMINISTRATIVE REVIEW RECORD

2555 ADMINISTRATIVE REVIEW RECORD 2555.1 The Department shall maintain a record for each administrative review offered or held. Each administrative review record shall include: (a) Documentation of the request for a fair hearing; (b) Documentation of the notice of the administrative review; (c) Evidence considered at the administrative review, if held; All status reports issued to the Office of Administrative Hearings; and All administrative review decisions issued.
