4221. REIMBURSEMENT: COMMUNITY TRANSITION SERVICES

4221 REIMBURSEMENT: COMMUNITY TRANSITION SERVICES 4221.1 Reimbursement for community transition services household set-up expenses specified under Subsection 4252.2 shall not exceed five thousand dollars ($5,000) per beneficiary per Waiver period. 4221.2 Reimbursement shall begin no more than sixty (60) calendar days before a beneficiary’s tentative discharge date and conclude no later than six (6) months after the beneficiary’s discharge from an institution or long-term care facility.
