402. APPEARANCE: REPRESENTATION

402 APPEARANCE: REPRESENTATION 402.1 Any person filing a request for a hearing may be represented by counsel or any other person as a representative. 402.2 On the first occasion of appearance, persons who appear in a representative capacity shall file a written notice of appearance. 402.3 The notice of appearance shall state the person's name, local address, and local telephone number. 402.4 The written notice of appearance shall be part of the record.
