1211. LOSS, THEFT, OR DESTRUCTION OF LICENSE

1211 LOSS, THEFT, OR DESTRUCTION OF LICENSE 1211.1 In case of the loss, theft, or destruction of any operator or vehicle license issued pursuant to this chapter, the licensee shall immediately notify the Office of the loss, theft, or destruction. 1211.2 Upon application made under oath on a form provided by the Office, and upon payment of the applicable fee, the issue a may duplicate license.
