1803 APPLICATION FOR AN ALARM AGENT LICENSE 1803.1 An application for an alarm agent's license shall be made on a form prescribed by the Director and shall contain a notarized statement that the information contained in the application is true and correct. 1803.2 Each application shall be accompanied by three (3) prints of a full-face photograph taken within not more than three (3) months prior to the date of the application.
