5408. REINSTATEMENT OF AN EXPIRED LICENSE

5408 REINSTATEMENT OF AN EXPIRED LICENSE 5408.1 A person whose license has been expired for no more than five (5) years may seek reinstatement of the license. A license shall not be eligible for reinstatement if more than five (5) years have elapsed since its expiration. 5408.2 To be eligible for license reinstatement, an applicant shall submit proof of having completed twenty-four (24) hours of continuing education meeting the requirements of ยง 5406.1 within the two (2) years immediately preceding the application date.
