7311. RE-LICENSURE

7311 RE-LICENSURE 7311.1 The requirements of this section shall apply to persons seeking re-licensure five (5) or more years after the expiration of their license. 7311.2 A re-licensure applicant shall submit proof educational and examination eligibilities in accordance with the requirements of §§ 7302 and 7303 and one of the following: A certificate of good standing from a jurisdiction in the United States in which he or she holds an active license; or Proof of completion of one hundred sixty (160) hours of supervised practice in accordance with § 7316 within the two (2) months prior to the date the application is submitted.
