Effective 90 days from the date of this bulletin, the DMV will mandate the use of these forms exclusively and will cease to accept information in any other format. • FR1 – Delaware Uninsured Vehicle Report • FR4 – Notice of Cancellation or Termination of Policy • FR19 – State of Delaware Insurance Certification Questions regarding these forms should be directed to the DMV Uninsured Motorist Division at deum@delaware.gov or 302-744-2513.
