5125.2.6 – “Stand-alone” or Isolated Concerns (Rev. 28, Issued: 10-21-16, Effective: 10-21-16, Implementation: 10-21-16) In situations when the practitioner(s) and/or provider(s) indicates that the concern is a “stand- alone” or isolated occurrence, a QIO may request any evidence the Quality Innovation Network -QIO determines is necessary and relevant to substantiate that an issue is an isolated occurrence (e.g., two to three examples of similar situations when the same concern did not present itself). In addition, the QIO shall review its own data.
