30 - Home Dialysis (Rev. 13599, Issued: 01-30-26, Effective: 05-01-26, Implementation: 05-01-26) Home dialysis is performed at an ESRD patient’s home, including in skilled nursing facilities and nursing facilities, by the ESRD patient or a caregiver who has completed an appropriate course of training as specified in 42 CFR § 494.100(a).
