140.4.2 - Respond to the CMS Office Requests for Information (Rev. 13825; Issued: 06-11-26; Effective: 07-13-26; Implementation: 07-13-26) Upon receiving notification of a new bankruptcy, the Contractor shall prepare a Bankruptcy Referral Checklist. The checklist is now divided into two (2) tiers, each designed to gather the bankruptcy information. The Contractor shall refer to Section 140.1 – Bankruptcy forms - for information required to be submitted to the CMS Office.
