(Before entering the Form CMS-116 data into the system, the SA verifies that the laboratory director is qualified. (See section 6006.7) • Form CMS-2567, Form CMS-670, and the Survey Specialty section are entered up to 45 days after the survey. • Certificate changes and updates entered up to 45 days after receipt by the SA.
