Physician Recertification

6.2.2 – Physician Recertification (Rev. 603, Issued: 07-21-15, Effective: 08-11-15, Implementation: 08-11-15) At the end of the 60-day episode, a decision must be made whether or not to recertify the patient for a subsequent 60-day episode. The plan of care must be reviewed and signed by the physician at least every 60 days when there is a need for continuous home care unless the beneficiary transfers to another HHA or the beneficiary is discharged and subsequently re-admitted (these situations trigger a new certification, rather than a recertification).
