CHAPTER 64B17-2 FEES 64B17-2.001 Fees 64B17-2.005 Biennial Renewal and Inactive Status; Delinquency; Reactivation; and Change of Status Fees (Repealed) 64B17-2.006 Duplicate License or Wall Certificate Fees (Repealed) 64B17-2.007 Certification of Public Records (Repealed) 64B17-2.001 Fees. (1) Each applicant for licensure shall pay a nonrefundable application fee payable to the Department of Health. An incomplete application shall expire 1 year after initial filing. Upon expiration of an incomplete application, an applicant must submit a new application and fee to be considered for licensure. (2) The application fee for licensure by taking the national examination or endorsement is $100. (3) When the Board certifies the applicant to sit for the examination, it is the applicantโ€™s responsibility to complete the examination process with the national vendor. Pursuant to the Americans with Disabilities Act, any applicant requesting special accommodations shall comply with Department of Health Rule 64B-1.005, F.A.C. (effective 11/5/14), which is incorporated by reference herein and may be obtained from the Board office or at http://www.flrules.org/Gateway/reference.asp?No=Ref-06100. (4) The initial licensure fee is $75. (5) The biennial renewal fee for an active license is $75. (6) The biennial renewal fee for an inactive license is $50. (7) The retired status fee is $50. (8) The delinquency fee is $55. (9) The fee for reactivation is $50. (10) The change of status fee is $40. (11) The duplicate license fee is $25. (12) The certification of a public record fee is $25. Duplicating fees as specified in Section 119.07(4), F.S., shall also apply.
