Licensees ordered to take and pass the Florida Laws and Rules Examination as a result of a disciplinary proceeding or reinstatement, must file DOH Form DH-MQA 1144, Final Order/Continuing Education Credit Florida Laws and Rules Application, revised 05/2021, which is incorporated by reference and available through http://www.flrules.org/Gateway/reference.asp?No=Ref-13420, or www.floridasphysicaltherapy.gov/resources.
