CHAPTER 64B20-5 RENEWALS, INACTIVATION, REACTIVATION 64B20-5.001 Renewal of Active Status License or Certificate 64B20-5.0021 Inactive Status License or Certificate 64B20-5.0022 Retired Status and Reactivation of Retired Status License; Fees 64B20-5.005 Requirements for Renewal of Provisional License 64B20-5.007 Delinquent Status License or Certificate (Repealed) 64B20-5.001 Renewal of Active Status License or Certificate. (1) Any person desiring to renew an active status license to practice speech-language pathology or audiology, or a certificate as a speech-language assistant or audiology assistant, shall apply to the Department of Health. The application for renewal shall be made on a form, prepared and furnished by the Department. (2) The Department shall renew only those licensees or certificate holders who have completed the application form, remitted the appropriate renewal fee established by Rule 64B20-3.007, F.A.C., and who have demonstrated that they have met the appropriate continuing education requirements established by Rule 64B20-6.001, F.A.C.
