CHAPTER 64B21-503 LICENSES 64B21-503.001 Reactivation of Inactive License 64B21-503.002 Renewal of an Active License 64B21-503.004 Retired Status License 64B21-503.005 Notice to the Department of Mailing Address and Place of Practice of Licensee 64B21-503.001 Reactivation of Inactive License. The Department of Health shall reactivate an inactive license upon receipt of the reactivation application, the biennial licensure fee as prescribed in Rule 64B21-501.003, F.A.C., the reactivation fee as prescribed in Rule 64B21-501.006, F.A.C., and upon the Departmentโ€™s certification that the licensee has satisfactorily submitted evidence of participation in fifteen (15) hours of approved continuing education for each year the license was on inactive status and thirty (30) hours of approved continuing education for the last biennium in which the licensee held an active license.
