CHAPTER 64B23-5 RENEWAL OF LICENSE, DELINQUENT LICENSE, REACTIVATION OF INACTIVE STATUS 64B23-5.002 Procedure for Reactivation of Inactive Status to Active Status 64B23-5.003 Delinquent License (Repealed) 64B23-5.002 Procedure for Reactivation of Inactive Status to Active Status. (1) A licensee may apply to reactivate an inactive license on Form #DH 1276-MQA, Application for Reactivation (9/2015), incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-06536. (2) No inactive license can be reactivated unless the licensee pays the applicable fees and demonstrates completion of at least 24 hours of approved continuing education for each biennium of inactive status.
