64B2-12.014 Supervising Physician Application Fee. (1) The fee for the initial application to supervise a certified chiropractic physician’s assistant shall be one hundred dollars ($100.00). (2) The fee for the biennial renewal certification to supervise a certified chiropractic physician’s assistant shall be one hundred dollars ($100.00).
