64B5-15.017 Application Fees for General Anesthesia, Pediatric Moderate Sedation, and Moderate Sedation Permits. (1) The application fee for a General Anesthesia Permit shall be $100.00. (2) The application fee for a Moderate Sedation Permit shall be $100.00. (3) The application fee for a Pediatric Moderate Sedation Permit shall be $100.00. (4) Application fees are not refundable.
